To change an active voice to a passive voice sentence is very easy, you just have to add a "by phrase" on it.

Example: He owns an expensive parker pen (active voice).

This expensive parker pen is owned by him.

Changing active voice to passive voice depends on the tense of the verb. 

The key phrase is "be + past participle of the verb".  

The passive voice does not always have "by ________ (doer of the action)" in the sentence.  Although to make sure, you may add "by _______(doer of the action).
Example of passive voice without "by ______".
Passive Voice:
    That temple was built in 5th century.  
    (It is not known who specifically built the bridge so the "by____" is
      omitted.)
